When couples are dealing with infertility, intrauterine insemination (IUI) is often the first recommended treatment option. How successful is the treatment? And are there any advantages of double IUI over single IUI?

What is IUI?

Intrauterine insemination (IUI) is a standard fertility treatment. The treatment involves placing sperm inside the woman’s uterus. The goal is to increase the chances of fertilization by increasing the number of sperm that reach the fallopian tubes. The treatment is usually offered as a first option for couples struggling with infertility. The choice is less invasive and expensive than some other fertility treatments, such as in vitro fertilization (IVF).

Good candidates

IUI is commonly recommended for couples with unexplained infertility, low sperm count, or decreased sperm mobility. The treatment is also beneficial for patients with cervical mucus problems. In contrast, IUI may not be the best choice for endometriosis, women who have a history of pelvic infections, or women who have a disease of the fallopian tubes.

What’s the difference between double vs single?

Often, IUI is only done one time in a woman’s monthly menstrual cycle. Typically, the only time is during ovulation. This is called a single IUI. In double IUI, however, women will get 2 inseminations on 2 consecutive days. Usually, the first is the day before ovulation, and the second is the day after ovulation.

What’s the advantage of double IUI?

Research is still ongoing regarding the pros and cons of single and double IUI. Some studies have shown that double IUI may increase the chances of conception in certain circumstances. Some of these circumstances include cases of male factor infertility. In particular, this practice may be beneficial when the sperm count is low.

What happens during IUI?

Before the IUI procedure, couples will typically have bloodwork done at the request of the fertility specialist. One the day of the appointment, the sperm sample is taken to a lab to be washed. This process concentrates the sperm. The sperm is then inserted into the uterus using a long, thin tube. Women are recommended to recline on the exam table for up to 30 minutes after the procedure. Most women experience no discomfort, although some mild cramping or bleeding is considered normal.
